General repair shelf
This shelf is located below
the general binding shelves. Use for all materials (except for
items in the locked stack) for which repair instructions are listed
on
the green repair slip. All information/instructions should be filled
out. If the necessary repair is not listed, place on the special
repair shelf.
Special repair shelf
This shelf is located across from Rich’s desk under the
notify shelves and next to the CDs. Use this shelf for materials
requiring a repair that is not listed on the green repair slip.
Fill out all the general information at the top; Rich will complete
the lower section.
Green repair slip
The green repair slip is for noting various types of damage to
materials that require that the item be sent to Repair. Anyone
may fill out this slip; please use it instead of a yellow routing
slip.
In completing the repair slip, please fill in as much information
as you have. Unless the item is not yet cataloged, this means the
entire upper half of the slip, excluding the high use/etc. area
which we generally leave blank. Guidelines for filling out specific
areas are given below.
Check the repair(s) required on the list given. If the necessary
repair is not listed, put on Rich’s special repair
shelf. He will complete the form.
Guidelines for filling out the slip
Call number: 1 section of the call number per line. The call number
does not include shelving locations.

Shelving location: This is the spine label location, not the Unicorn
location.

Title/Author: Give the last name of the author only (unless you
feel it needs to be more specific) and the title from the t.p.
or 245 of the bib record. There is no need to worry about consistency.

Repair instructions: Most are self-explanatory. A couple clarifications:
Pam bind: Only for items larger than 34 x 35 cm. Other pam binds
go directly on the pam bind shelf.
Tip-in: Use only for actual repairs or new bindings with more
than 2 pages of tip-ins; tip-ins of 1-2 pages in new bindings can
go directly to End Processing.
